Application Instructions

To ensure that your application is processed immediately:

• Please fill in each blank on the application, including witness signatures on both pages.
• Once everything is filled out please fax application (including copy of state tax exempt card) back to 410-646-3745 attention Falter Fund Raising Elizabeth or Patrice.
• After you fax the application, please call us at 410-646-3641 to ensure that everything is filled in, etc.
• After you have called us, please make a copy for your records and mail the original application to: 3501 Benson Ave. Baltimore, MD 21227.

Once we have received your application, it takes approximately two days for public schools and five to seven days for organizations for the accounting department to completely set up a customer number and 30 day credit terms. In the case of organizations, we cannot grant you terms until we receive the original application in the mail, so please mail it as soon as possible.
