Application Instructions

To ensure that your application is processed immediately:

• Please fill in each blank on the application, including witness signatures on both pages.
• Once everything is filled out please fax application back to 410-646-3745, attention Falter Fundraising.
• Please allow approximately two days for public schools and five to seven days for organizations to process the application.
• We will call with your new customer number and approved terms once the process is complete.
